If you want to stay as close to OEM as possible, you have to check these guys out. Many swear by them. The hardest part about the task ahead, is cleaning out your fork. I'm sure MTBR or RetroBike has more info on this..

Here's a good scan of the "Totally Driven" logo, which you can remove the triangle and you've got a perfectly prepped downtube decal. Insert color gradient of your choice and voila!

Yep, scan those at high resolution, then anyone with Illustrator or Photoshop experience can create a custom set for you, using the colours of your choice.

Looks like a 12 to me, but Muhich rode a 14 and her top tube was almost straight with the seat stays, so I'm on the fence..

It wouldn't be a Herting frame, 3D didn't fabricate anything except for some prototypes and race frames, so rule them out.
Yeti was crafting early frames, but I'm told that there could have been a contract with a company called Framesmith for a limited time. I'll keep digging.
